Let's start with the question that's been posed - verbatim:
How do you get your teeth so pearly?

On one hand, it may be an enquiry as to how it was done, like on a technical or practical level?
What means were employed? Bleach? Like assholes? Painted? Corroded? There's probably many ways to achieve such a result.
Some may involve tooth ache. Some other pain. Or pains.
On the other hand, it might be a different use of the word 'how'. The way the Englishman's northern neighbours tend to use the word.
Thus reads the question: Why, cunt, d'you get your pearlies so white?
Depending on your lifestyle choices, especially those common in the area, that's quite a hassle...
Tell me it's worth it. Help me understand.
Now, as for the proposed response; well, there isn't actually one. Not per se. But hinted at? Implied? Written. Sung. Between the lines?
